At   21st Century Care Solutions, our services are individually tailored specifically to each person's unique situation and levels of need.
We begin with an individualized assessment of a person's needs and goals, and then we create a Care Plan, which is like a road map whereby we address all of the issues that were noted in the assessment and we work to bring solutions.
 Penny's care has been a very difficult case from the beginning. We put our trust and Penny's care in 21st Century and  we can't imagine anyone doing a better job.  Every person on staff is a true professional and has our utmost respect. The big winner has been Penny.

Assessment and Problem Solving
In the Assessment, we examine the whole range of issues in a person's life that has impact on a person's well-being. This includes but is not limited to: safety and fall prevention, transportation needs, cognitive health, daily care needs, nutritional concerns, health (physical, emotional, and psychological), social/relationship challenges, advocacy needs with medical providers.     
Development of a Care Plan
A care plan is developed based on the assessment information. This plan gets modified, in consultation with the client, the Power of Attorney, Trustee, or Conservator and/or the family, as circumstances change and continues over the life cycle of the person's life, or for so long as the Care Manager is engaged.  A  Care Plan can be developed to meet Probate Court requirements as well. 
What types of other services does a Care Manager provide?
  • Arrange for home services & monitor quality of care             
  • Resolve family conflicts and other family issues relating to long term care
  • Oversight and support to paid caregivers
  • Assistance with home-type/community placements 
  • Find appropriate solutions to avoid a crisis (fall prevention, home safety, proper nutrition, medication oversight) 
  • Coordinate medical appointments and medical information.
  • Assist families in positive decision making
  • Maintain a continuum of communication between client and members of his or her support network so that the client does not fall through the cracks.

Services are consistently tailored to the individual needs identified in the care plan.  As needs change the care plan changes.  We are there to notice, and immediately intervene.
